Add/Edit Vaccine for Reports Screen

 Purpose

The Add/Edit Vaccine for Reports screen is used to add/edit vaccines for Georgia-specific reports: GANABRE2, GANABMAN, PRIACCT1, and DPHACCT1.

 Functionality

Button

Description

When the  Save  button is clicked, the Vaccine Name, Months Factor, State Supplied option, Trade Name Listing are Saved. The Vaccine Name, Months Factor, and State Supplied option can be saved without with no trade names in the Trade Name Listing.

When the  Cancel  button is clicked, the key data fields will not be examined for data validity, and the vaccine data will not be saved to the database. Instead, the Manage Report Defaults screen will be displayed.

When the  New  button is clicked, all the Trade Name fields will be cleared for entering a new Trade Name. Nothing is saved to the Trade Name Listing when the  New  button is clicked.

When the  Next  button is clicked, all Trade Name information fields will be examined for validity then added to the Trade Name Listing, and the Trade Name fields will be cleared for entering another Trade Name. After the desired Trade Names have been added to the Trade Name Listing, Click the  Save  button to save the information to the database.

When the  Delete  button is clicked, the Trade Name selected from the Trade Name Listing is deleted and the Trade Name fields will be cleared for entering a Trade Name. After the desired Trade Names have been deleted from the Trade Name Listing, Click the  Save  button to save the information to the database.

 Required Fields

Field Name

Description

Vaccine

This is the selected vaccine name to be displayed on reports. Assign or change the vaccine name by typing the desired name into the 'Vaccine Name' text box. 'Relation' pick list. * Note *

Months Factor

This is the Months Factor, which determines how many months supply of the currently selected vaccine a site should have available in their office. Assign or change the Months Factor by typing the desired factor into the 'MOnths Factor' text box.

State Supplied?

This indicates whether or not the currently selected vaccine is supplied by the state. To indicated if a vaccine is state supplied, select Yes or No from the 'State Supplied?' options.

Trade Name

This is the state in which the selected responsible person currently lives. Assign or change a Trade Name by selecting the desired Trade Name from the 'Trade Name' pick list.

Cost per dose

This is the cost per dose for the Trade Name that is currently selected. lives. Assign or change the Cost per dose by typing the desired cost into the 'Cost per dose' text box.

Field Name

Description

Manufacturer

This is the selected Trade Name's manufacturer. Assign or change the manufacturer by selecting the desired manufacturer from the 'Manufacturer' pick list.

Minimum Order

This is the selected Trade Name's minimum order. Minimum Order is normally the Package Size multiplied by the Vial Size. Assign or change the minimum order by typing the desired number into the 'Minimum Order' text box.

Package Size

This is the selected Trade Name's package size, which is the number of vials in a package. Assign or change the package size by typing the desired number into the 'Package Size' text box.

Vial Size

This is the selected Trade Name's vial size, which is the number of doses per vial. Assign or change the vial size by typing the the desired number into the 'Vial Size' text box.

 Notes

In order for the registry to create and maintain a list of vaccines for reports there is a minimum amount of information necessary that must be provided by a user. These select pieces of information are the Required Fields and are denoted in the registry by the blue information field labels as seen in the example below.
