Purpose |
The Blank Forms screen is used to display and print a blank Certificate of Immunization (Form 3231), and/or a blank University System of Georgia Certificate of Immunization (USG Form). These forms may then be filled in with the necessary information for a client to attend any daycare, childcare, nursery or school.

Functionality |
The Blank Forms screen is accessible from the Menu Panel/Status Bar.
Generating Reports
To produce a Form 3231 or USG Form, click on either the Form 3231 or USG Form hyperlink.
After the system generates the selected report, it will automatically be displayed using the Adobe Acrobat Reader. *Note*

This report displays the immunization history for a child attending any school or daycare facility. |

This report displays the immunization history for a person attending an institution of the University System of Georgia. |

If your computer does not Adobe Acrobat Reader version 5.0 or greater installed, please see the " **REPORT VIEWING REQUIREMENTS** " notification on the Blank Forms screen, or contact your administrator.
