| Opt Out/Opt In Forms Screen |

Purpose |
The Opt Out/Opt In Forms screen is used to display and print a blank Opt Out Form (Form-GRITS008), and/or a blank Opt In Form (Form GRITS007). These forms may then be filled in with the necessary information for a client to opt out of or opt into the registry.

Functionality |
The Opt Out/Opt In Forms screen is accessible from the Menu Panel/Status Bar.
Generating Reports
To produce Opt Out Form or Opt In Form, click on either the Opt Out Form or Opt In Form hyperlink.
After the system generates the selected form, it will automatically be displayed using the Adobe Acrobat Reader. *Note*

Opt Out Form |
The Opt Out Form is required by Georgia to allow an individual to request that a person's immunization history be removed from the registry and no further immunization data be accepted into the registry. |

Opt In Form |
The Opt In Form is required to allow a person who has previously opted out of the registry to opt back into the registry thereby allowing collection of immunization data on the person. |

If the computer does not have Adobe Acrobat Reader version 4.0 installed on it, then please see the " **REPORT VIEWING REQUIREMENTS** " notification on the Opt Out/Opt In Forms screen, or contact your administrator.
